Alternate Level of Care (ALC) Patient Experience

Almost 10% of patients admitted to St. Joe's Hospital in Hamilton are unable to go home after they are clinically stable because they require additional resources. The majority of these "Alternate Level of Care" patients are waiting for long term care.

 

This design project, led by graduate students at McMaster University in collaboration with St. Joseph's Healthcare Hamilton, aims to improve the ALC patient experience by promoting patients' ability to make informed decisions, supporting healthcare staff during discharge planning, and through better use of hospital resources.

As Faculty Lead, I guide students through a human-centred design process to understand how we can best meet the needs of patients, family members, as well as members of the care team both at St. Joe's and at other community partner organizations.
